We are required by law to keep your health information private and secure, to tell you promptly if it is ever exposed, and to follow the practices in this notice.

Ask for our privacy officer at the front desk, or call or email us. You can also file a complaint with the U.S. Department of Health and Human Services. We will never act against you for filing a complaint.
